Terms and Conditions of Money Back Guarantee
Affinity4 will refund all the up-front charges you paid, including the activation fee, monthly
service charges, shipping and handling, taxes and regulatory fees.
Our money back guarantee applies only to the first line per account, not to additional or
secondary lines. The guarantee does not apply to any charges for long-distance usage, international
usage, directory assistance or operator-assisted calls, or payphone calls to your Digital
phone.
To be eligible for the refund, all of the following conditions must be met:
- Cancellation request must be received within 30 days of activation date. The activation date is
exactly 5 days from the date you ordered service. It is neither the day you first used the service
nor the day you received the equipment.
- We receive your ATA device back, in its original condition, reasonable wear and tear excluded,
no more than 14 days after you cancel service.
- Before returning the device, you must contact Affinity4 Customer Support to obtain an authorized RMA (Return to Manufacturer Authorization). This number must be placed on the outside of the box.
You can reach Customer Support at 1-800-705-9341. They will provide instructions on packaging and shipping the ATA device back. - You pay the cost of shipping the ATA back to us.
Please note: If you do not ship the ATA device back to us, you will be assessed a $99 charge to pay for the device.
